Make-up Lab

This is NOT an EXTRA CREDIT make-up lab! This lab will count as a regular make-up lab only if you need to make one up.  In order to receive credit, you MUST do the following:
  1. Fill out the form below to register for the make-up lab. I will only accept labs from those students who register here in my website.
  2. Print out the 3 pages of this lab and answer all questions DIRECTLY on the paper.  You can write short answers on the back if you need more room. I am only accepting these three sheets so make sure to answer everything on these sheets of paper!
  3. CHECK THE BOX BELOW STATING THAT YOU UNDERSTAND ALL OF THESE RULES.
